You can receive MD24 Cardiovascular care if you:

Have received a new diagnosis of heart disease, i.e atrial fibrillation, heart failure, etc.
Have been hospitalized one or more times in the past 12 months
Have frequent medication changes
Have more than one chronic disease process
Have a history of poor medication compliance
Chronic high blood pressures, history of smoking, and family history of cardiovascular diseases.

We accept patient self-referrals or referral from your primary or specialty physician. Our office staff will verify the level of insurance coverage prior to beginning the treatment program
